Participants will receive practical, scenario-based instruction in workplace accident investigation and OSHA injury and illness recordkeeping.
Training Details
Format: Live Virtual Instructor-Led Training via Zoom
Cost: FREE
Registration: Eventbrite registration is required
Participants must attend using the same full name listed on their Eventbrite registration. Their full name must also be displayed on their Zoom screen to remain in the class and qualify for a certificate.
What You Will Learn
Participants will learn how to:
- Investigate workplace accidents and incidents
- Identify facts and possible root causes
- Recommend corrective actions
- Complete OSHA injury and illness recordkeeping forms
- Apply the information to realistic workplace scenarios
Forms Used During Training
Participants will receive hands-on practice completing:
- UC San Diego Accident Investigation Form
- OSHA 300 Log
- OSHA 300A Annual Summary
- OSHA 301 Incident Report
These forms will be listed on the Certificate of Completion as demonstrated competencies.

Certificate Requirements
To receive a Certificate of Completion, participants must:

- OSHA 300A Annual Summary
- OSHA 301 Incident Report
Completed documents must be emailed to the submission address provided during the training.
